Job summary

Liberty Home Care is seeking an experienced Home Care Administrator/Clinical Manager to supervise branch operations, coordinate delivery of home health services, and ensure compliant documentation and staffing. This role emphasizes leadership in clinical processes, program efficiency, and staff development.

The position requires current RN licensure in the branch state, with 5+ years of clinical nursing and 3+ years in home health preferred.

Qualifications

• Requires current RN licensure in the state of the branch.
• Five years of clinical nursing experience across settings.
• Three years of home health experience preferred.
• Knowledge of home health regulations and financial management.
• Strong communication and interpersonal skills; fluent English.

Responsibilities

• Supervise operational processes of the sub-unit/branch.
• Coordinate delivery of home health services as a resource for staff and providers.
• Monitor clinical processes, documentation and financial data.
• Oversee case management and patient services.
• Ensure documentation for patient care and staff performance.
• Perform Start of Care and recertification record reviews.
• Ensure accurate coding of patient medical records.
• Supervise direct care staff, schedulers, OSTs and Homecare Liaisons.
• Provide direct patient care as needed and allocate staff to cases.
• Participate in orientation, competency evaluation and training.
• Perform HR functions per agency policy to ensure compliance with regulations.
• Perform additional duties as assigned.
