Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes per a care plan. Responsibilities include personal care, daily living activities, and documentation of patient condition.

You will work part-time with flexible hours, and must maintain CPR certification and follow state requirements. The team emphasizes quality care and compassionate support for patients and families.

Qualifications

• Meets training requirements per state and federal laws when applicable.
• Ability to read and follow written instructions and document care given.
• Must maintain CPR certification per state regulations.
• PPD or chest X-ray if applicable (as required).
• Complete Maxim competency program as required.

Responsibilities

• Assist patients with daily living according to the plan of care (baths, oral hygiene, dressing, toileting, feeding).
• Take and record vital signs as ordered (temperature, pulse, respiration, blood pressure).
• Assist with safe transfers and ambulation.
• Support exercise as outlined in the plan of care.
• Perform ROM and simple procedures as ordered by care plan.
• Handle patient laundry.
• Ensure safety and proper use of equipment (foot stools, side rails).
• Observe home safety and educate on safety practices.
• Recognize emergencies and follow emergency management procedures.
• Follow the plan of care and complete all assigned tasks.
• Communicate clearly with client, family/caregiver and office.

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable

• Ability to read, follow written instructions and document care given

• Possess and maintains current CPR certification in accordance with state regulation

• PPD or chest x-ray if applicable

• Successful completion of Maxim screening examinations

• Participation and completion of Maxim’s competency program
