Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan. The role is part-time with flexible hours and a competitive salary.

Responsibilities include assisting with hygiene, transfers, activities of daily living, and documenting patient conditions for the supervising nurse. CPR certification and state-compliant training are required to ensure safe, high-quality care.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Maintain CPR certification as required by state regulation.
• Pass Maxim screening examinations and competency program.

Responsibilities

• Assist with baths, oral hygiene, dressing, toileting, feeding per plan of care.
• Take and record oral, rectal, axillary temperatures, pulse, respiration and blood pressure as ordered.
• Safe transfer and ambulation according to the plan of care.
• Assist with exercises and ROM as ordered.
• Handle patient laundry and ensure safety with equipment.
• Observe and document patient condition and report changes to the assigned nurse supervisor.
• Follow the plan of care and complete all tasks.
• Communicate clearly with client, family/caregiver and office.
• Identify and respond to emergencies per procedures.

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable

• Ability to read, follow written instructions and document care given

• Possess and maintains current CPR certification in accordance with state regulation

• PPD or chest x-ray if applicable

• Successful completion of Maxim screening examinations

• Participation and completion of Maxim’s competency program
