Professional Case Management is hiring Home Health Aides/ CNAs to deliver in-home care across Ohio. You’ll provide one-on-one support, help patients maintain independence, and contribute to improving health and quality of life for those who served our country.

Qualifikationen

• HHA or similar experience required; must pass competency assessment with RN.
• TB test; reimbursement may be available.
• Reliable transportation to perform in-home visits.

Aufgaben

• Provide in-home health care to patients in their homes.
• Develop one-on-one patient relationships and monitor health needs.
• Assist with activities of daily living and light housekeeping as appropriate.
