As a Home Health Aide (HHA) you are an essential member of the home care team who works under the supervision of a nurse. You provide compassionate support with activities of daily living and basic personal care needs while ensuring a safe, comfortable living environment. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey.

HOW YOU'LL WORK:
You'll perform personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene. You will effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team.
You'll be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel.
MAJOR AREAS OF RESPONSIBILITY:
Patient Care: Assist patients as needed with lifting and transferring
Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the case manager.
Minimize Patient Risk: Ensure the living environment is healthy and safe.
Policies: Completes all documentation following agency protocol and Medicare/Federal guidelines.
Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude
Patience is a virtue when working with patients, families, physicians, and coworkers
Attention to detail is critical, as is being observant and following directions
REQUIREMENTS:
High school diploma or equivalent.
State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA).
One year of experience as a nurse aide in a hospital, nursing home, home care agency, or a private setting under the direct supervision of a RN, which can be verified.
Completion of skills competency test with 80% or higher score.
Valid driver's license and auto insurance in your name as a driver
Capable of all physical demands
