CenterWell Home Health seeks a dedicated Home Health Aide to support patients in their homes under RN supervision. You will perform direct care, assist with medications, monitor vitals, and help with daily activities to promote rehabilitation and independence.

Requirements include completing a CNA/HHA program within 24 months, valid state certification, and current HHA/CNA credential. A valid driver's license and reliable transportation are required.

Qualifications

• Must meet applicable state certification requirements.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsabilités

• Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist with self-administered medications and observe medication administration.
• Take vitals and report abnormal results to supervisor.
• Support personal hygiene, feeding, mobility, and daily living activities.
• Document care provided and communicate with caregivers and staff.
