VitalCaring Group is seeking a full-time Aide to join our home health and hospice team in Oklahoma. You will work with the RN to implement patient care plans, provide hands-on personal care, and help with transfers and ambulation, all while documenting daily care in our EMR system.

Qualifications

• High school diploma or equivalent preferred.
• State certification required for state-specific aide certification.
• State licensure or competency evaluation program may be required.
• One year experience as a Home Health or Hospice Aide or Nursing Assistant preferred.

Responsibilities

• Work with the RN to follow the patient care plan.
• Collaborate with the care team to meet patient needs.
• Provide hands-on personal care and assist with safe transfers and ambulation.
• Document care delivery daily in the Electronic Medical Record system.

As the Aide, you will:

• Work in collaboration with the RN to fulfill the defined patient-specific Aide care plan
• Collaborate with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed
• Deliver hands‑on personal care and supportive services to assist with safe transfers and ambulation
• Thoroughly document care delivery daily in our Electronic Medical Record system
• Contribute to a culture of caring through individual accountability and teamwork

Experience to Deliver on our Mission:

• High school diploma or equivalent preferred
• Certification required for State-specific certified aide certification
• State licensure or a competency evaluation program, valid state driver’s license and auto liability insurance
• One year experience as a Home Health or Hospice Aide or Nursing Assistant in a hospital, nursing home, or home health agency. Home health experience preferred
