Job summary

VitalCaring Group is seeking a full-time Home Health or Hospice Aide to deliver compassionate personal care. You will collaborate with the RN to implement patient care plans, assist with transfers, and document care in the EMR system. The role emphasizes teamwork and accountability within a supportive environment.

Qualifications include a high school diploma or equivalent, state-certified aide certification, and a valid driver’s license. One year of home health or hospice experience is preferred.

Qualifications

• High school diploma or equivalent preferred.
• Certification required for State-specific certified aide certification.
• State licensure or a competency evaluation program, valid state driver’s license and auto liability insurance.
• One year experience as a Home Health or Hospice Aide or Nursing Assistant in a hospital, nursing home, or home health agency.

Responsibilities

• Collaborate with RN to fulfill patient-specific Aide care plan.
• Assist with safe transfers and ambulation and provide hands-on personal care.
• Document care delivery daily in the EMR system.
• Support care team as an extension of nursing or therapy services.
• Contribute to a culture of caring through accountability and teamwork.

As the Aide, you will:

• Work in collaboration with the RN to fulfill the defined patient-specific Aide care plan
• Collaborate with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed
• Deliver hands-on personal care and supportive services to assist with safe transfers and ambulation
• Thoroughly document care delivery daily in our Electronic Medical Record system
• Contribute to a culture of caring through individual accountability and teamwork
