A leading healthcare provider is seeking Certified Home Health Aides for per diem positions in Sacramento. The CHHA will deliver essential care to patients in their homes, focusing on personal care, support, and maintaining patient comfort. Ideal candidates should have a California CHHA certification and strong interpersonal skills, ensuring quality care is delivered with compassion and professionalism.
The UC Davis Health Home Care Services Certified Home Health Aide (per diem) is an integral part of an interdisciplinary team providing care to chronic and terminally ill patients in their home. Home-based care includes intermittent visits and shift care. The Certified Home Health Aide (CHHA) reports directly to the Therapies and Home Health Aides Supervisor with clinical supervision provided by either the nursing or therapist case manager for the patients receiving care. The CHHA works within the scope of licensure developed by the California Department of Health Services, providing a full range of specialized care focused on increasing comfort, providing personal care/hygiene, maintaining independence, and supporting a safe, healthy environment for the hospice and/or home care patient.

Department Overview
The UC Davis Home Care Services Home Health program is a full-service agency offering the best in home health care. Our Home Health program is nationally ranked among the highest in patient satisfaction. The Home Care Services department is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care. This position is part of the Hospice Program (home care).
Department Specific Job Scope
The incumbent primarily works in the field (90%) as this is an in-home care position. Occasionally (10%), the incumbent works in the HCS departmental office located in Rancho Cordova for meetings, charting, and additional, administrative tasks.

UC Davis is a smoke and tobacco free campus effective January 1, 2014. Work environment may include clients' homes, hospitals, care facilities, and clinics. Travel distance may vary based on client location. Must participate in the DMV pull notice system. Auto insurance must comply with UCDH Travel Regulations Policy G-28, pg. 18, item iii; Insurance Coverage: 50/100/50. Must be able to work occasional evenings, overtime, on-call, weekends, and holidays as defined by the collective bargaining agreements (if applicable).
