Community Care, Inc. is currently seeking a dependable home health aide or CNA to provide one-on-one in-home care to a 17-year-old young man in the Monongahela area.
Daylight shifts available.
Overtime is available but not required.
Position Summary
The home health aide (HHA) is a paraprofessional responsible for providing personal care and assistance with activities of daily living, exercise programs, and related services in the home for the sick and disabled, especially when no family member can assume this responsibility. This role functions under the direction, instruction, and supervision of a registered nurse and/or therapist.
Clinical Skills
- Assist with personal hygiene tasks such as bathing, nail and skin care, oral hygiene, and shampooing.
- Prepare nutritious meals or snacks for the patient, as assigned.
- Assist with treatments like dressings, range of motion exercises, ambulation, and routine exercises.
- Provide basic emotional support and help manage difficult behaviors.
- Maintain a clean, safe, and healthy environment for the patient.
- Make pertinent observations, recognize emergencies, and seek guidance appropriately.
- Remind the patient to take medications as per the care plan (note: do not administer medications).
- Coordinate with other care providers as directed by the nurse or therapist.
- Recognize and report abuse or neglect.
- Escort the patient safely to medical appointments, social activities, and shopping.
Documentation
- Follow the HHA Plan of Care accurately.
- Complete and submit home health aide notes/flow sheets for each visit within the electronic medical record.
Communication
- Report any changes in the patient's condition or home situation to the RN supervisor or care manager.
- Contact the RN supervisor or designee for immediate clinical needs.
- Address scheduling or non-clinical issues with the scheduling coordinator.
- Maintain confidentiality adhering to HIPAA regulations.
- Submit monthly availability for scheduling.

- Follow infection control and safety policies.
- Adhere to all agency policies to maintain standards of care.
- Perform only trained activities and report any uncertainties to the supervisor.
- Maintain professionalism and a positive image of the agency.
- Provide care in a compassionate, therapeutic, and professional manner.
Professional Development
- Attend at least twelve hours of health-related continuing education annually.
- Participate in mandatory annual in-service sessions.
- Engage in staff meetings, care conferences, educational sessions, and performance improvement activities as appropriate.
Additional Duties
Perform any other duties as assigned.
Qualifications
- Must be at least 18 years old; high school diploma or GED preferred.
- Successfully pass a competency evaluation; completion of a formal HHA or CNA program is preferred.
- Score at least 70% on the HHA pre-employment test.
- One year of supervised experience as an HHA or CNA is preferred.
- Ability to understand and follow directions and maintain appropriate records.
- CPR certification required.
- Reliable transportation is required if traveling between assignments on the same day; valid driver’s license and insurance are necessary.
- Clearance of criminal background checks and regulatory requirements such as Childline, FBI, OIG, Medicheck, EPLS, and others as needed.
