We are seeking a Home Health Aide (CNA required) to join our Penn Home Care team in Southwest Philadelphia. This part-time role involves assisting patients with personal care, activities of daily living, and home health visits.

Responsibilities
  • Assist patients with bathing, grooming, and skin/hair care.
  • Prepare light meals and assist with eating.
  • Help patients dress and use assistive devices like bedpans and urinals.
  • Support patients with exercises and transfers for comfort and mobility.
  • Perform household tasks as per care plan and help with medication reminders.
  • Accurately document patient care and report any changes or issues.
  • Communicate effectively with patients' families, nurses, and coordinators.
  • Follow the care plan and review it bi-weekly.
  • Use personal transportation for patient visits, meeting driving requirements.
Qualifications
  • Certified Nurse Aide (CNA) certification or student nursing assistant in PA with relevant coursework and GPA requirements.
  • Valid Driver's License and Car Insurance.
  • High School Diploma or GED.
  • 1+ years recent experience as a Nursing Assistant or Home Health Aide, or 0-1 years with specific training.
Additional Requirements
  • Attend annual training and complete 12 hours of continuing education annually.
  • Maintain all required competencies and follow procedures.
