Job summary

Our Hospice of South-Central Indiana is seeking a Day-shift Home Health Aide (HHA) for the 14-bed Inpatient Facility in Columbus, IN. The HHA will handle a patient caseload under the supervision of an RN Case Manager, providing non-skilled nursing assistance and patient comfort.

Shifts are day 7:00am–7:30pm, 3x12s, with every other weekend. Salary range is $20.00 to $21.50 per hour, plus shift premium and other company guidelines. PSLF eligibility is available for Our Hospice employees.

Qualifications

• High school diploma or GED (or equivalent).
• Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in Indiana.
• Interest and ability in providing patient care.

Responsibilities

• Provide routine patient care in the IPF as assigned, with comfort, care, and compassion.
• Assist patients with daily hygiene activities (showering, dressing, shaving, toileting).
• Observe patients and report observations to the RN Case Manager or Nurse Facilitator.
• Complete patient records accurately in HCHB and timely progress notes.
• Attend and participate in Interdisciplinary Group meetings and staff trainings.
• Participate in at least 12 hours of in-service education per year.
• Be willing and able to lift patients for routine hygiene as needed.

Our Hospice is looking for a day-shift Home Health Aides (HHA) for our 14 bed Inpatient Facility (IPF) in Columbus, IN. This person works with a caseload of patients alongside a RN Case Manager. Our patients and families are in the most important time of their life and need non-skilled nursing to help navigate their pathway. This position pays premiums for the night shift.

• Shift Available: Day shift, 3x12s and every other weekend; 7:00am - 7:30pm

An HHA at Our Hospice Will:

• Provide routine patient care in the IPF per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families.
• Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet).
• Observe patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
• Complete patient records accurately in Home Care Home Base (HCHB) and on a timely basis.
• Attend and participates in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
• Participate in a minimum of twelve (12) hours in-service education per year.
• Be willing and able to lift patients for routine hygiene as needed

What an HHA at Our Hospice Needs:

• High school diploma or general education degree (GED)
• Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana
• Interest and ability in providing patient care

Salary Range: $20/hr - $21.50/hr plus shift differential and premiums based on company guidelines
