Position Overview

Safe Life Home Health is seeking dedicated CNAs to join our team and make a meaningful impact in our community. As a CNA at Safe Life HHC, you'll play a crucial role in providing compassionate care to our home health patients. Your responsibilities will include assisting patients with their daily activities, ensuring their comfort, and supporting their overall well-being.

Responsibilities

  1. Assist with Activities of Daily Living (ADLs): Provide assistance with bathing, dressing, grooming, and toileting for home health patients.
  2. Mobility Support: Help patients with mobility by assisting them with walking, transferring, and using mobility aids.
  3. Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.
  4. Medication Reminders: Assist patients with medication reminders and ensure compliance with medication schedules.
  5. Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
  6. Hygiene and Comfort: Maintain patient hygiene and comfort, including changing bed linens and providing oral care.
  7. Observation and Reporting: Observe and report changes in the patient's condition, behavior, or symptoms to the nursing team.
  8. Documentation: Accurately document care provided, vital signs, and any changes in the patient's condition in the EMR system (Kinnser by WellSky).
  9. Communication: Maintain clear and effective communication with the patient, their family, and the healthcare team.
  10. Safety Measures: Ensure a safe environment for the patient by following infection control and safety protocols.
  11. Transportation: Provide transportation to medical appointments or assist with mobility as needed.
  12. Emotional Support: Offer emotional support and companionship to patients, promoting their overall well-being.
  13. Collaboration: Collaborate with the healthcare team, including nurses and therapists, to provide comprehensive care to patients.
  14. Patient Education: Educate patients and their families on self-care and safety measures.
  15. Compassion: Show compassion, kindness, and empathy while providing care to home health patients, who may be dealing with pain or illness.

Qualification

  • Active CNA certificate
  • Valid driver's license and car insurance
  • CPR certification
  • Prior experience in home health is a plus.
  • Familiarity with EMR Kinnser by WellSky is advantageous.
  • Excellent communication skills
  • Compassion and kindness are essential qualities.
