Adaptive Home Health is hiring field-based caregivers to provide personal care in patients’ homes. You will assist with daily living activities, mobility, and medication reminders while maintaining safety and dignity.

We offer competitive hourly pay and mileage reimbursement, with a full-time schedule and a focus on reducing administrative burden through technology. You will work closely with a supervising nurse to update care plans and ensure high-quality patient care.

Qualifications

• Current Texas HHA certification.
• 1+ years of personal care experience.
• CPR/BLS certification.
• Current Texas driver’s license and reliable transportation.
• Ability to lift and transfer safely.
• Strong communication skills and professional demeanor.
• High school diploma or GED.

Responsibilities

• Provide personal care including bathing, dressing, grooming, and toileting.
• Assist patients with transfers, ambulation, and mobility safely.
• Support medication reminders as directed by the care plan.
• Monitor and report vital signs and daily living activities.
• Perform light housekeeping related to safety and care.
• Document all care in Homecare Homebase (HCHB).
• Communicate with patients, families, and the care team to ensure continuity of care.
• Follow infection control practices and protect patient privacy.
• Assist with range of motion and restorative exercises as ordered.
• Coordinate with supervising RN/LVN for care plan updates.

• Comp: $17.00 - $18.00 per hour + mileage reimbursement

What you will do:

• Provide personal care assistance including bathing, dressing, grooming, and toileting.

• Assist patients with safe transfers, ambulation, and mobility using proper body mechanics.

• Support patients with medication reminders as directed by the care plan.

• Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse.

• Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining clean environment).

• Document all care provided accurately and timely in Homecare Homebase (HCHB).

• Communicate clearly with patients, families, and the care team to ensure continuity of care.

• Follow infection control practices and maintain patient dignity and privacy.

• Assist with range of motion exercises and other restorative care activities as ordered.

• Coordinate with supervising RN/LVN for care plan updates and patient needs.

Requirements

Must-haves

• Current Texas HHA certification

• 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)

• CPR/BLS certification

• Current Texas driver’s license and reliable transportation

• Ability to lift, transfer, and provide hands-on care safely

• Strong communication skills and professional demeanor

• Comfortable working independently in patients’ homes

• High school diploma or GED

Nice-to-haves

• Previous home health experience

• CNA or other clinical training/certification

• Experience with medically complex or high-acuity patients

• Bilingual (English/Spanish)
