Job summary

Adaptive Home Health is seeking a Field-based Home Health Aide in the Las Vegas area to provide compassionate personal care in patients' homes. You will assist with ADLs, mobility, and medication reminders while coordinating with the supervising nurse to update care plans and ensure safety.

The role offers full-time hours (Mon-Fri with some weekends as needed), a competitive hourly pay of $17–$18 plus mileage reimbursement, and access to technology that reduces documentation burden and improves

Qualifications

• Vegas HHA certification is required.
• 1+ year of personal care experience in home health or a similar setting.
• CPR/BLS certification is required.
• Valid Vegas driver's license and reliable transportation.
• Ability to lift, transfer, and provide hands-on care safely.
• Strong communication skills and professional demeanor.
• Ability to work independently in patients' homes.
• High school diploma or GED.

Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, and toileting.
• Assist patients with safe transfers, ambulation, and mobility using proper body mechanics.
• Support patients with medication reminders as directed by the care plan.
• Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse.
• Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining clean environment).
• Document all care provided accurately and timely in Homecare Homebase (HCHB).
• Communicate clearly with patients, families, and the care team to ensure continuity of care.
• Follow infection control practices and maintain patient dignity and privacy.
• Assist with range of motion exercises and other restorative care activities as ordered.
• Coordinate with supervising RN/LVN for care plan updates and patient needs.

• Comp: $17.00 - $18.00 per hour + mileage reimbursement

Requirements

Must-haves

• Current Vegas HHA certification

• 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)

• CPR/BLS certification

• Current Vegas driver's license and reliable transportation

• Ability to lift, transfer, and provide hands-on care safely

• Strong communication skills and professional demeanor

• Comfortable working independently in patients' homes

• High school diploma or GED

Nice-to-haves

• Previous home health experience

• Familiarity with Homecare Homebase (HCHB) or similar EMR

• CNA or other clinical training/certification

• Experience with medically complex or high-acuity patients
