A leading healthcare provider in Michigan is seeking a Home Health Aide/Certified Nursing Assistant (HHA/CNA) to provide compassionate care to clients in their homes. The ideal candidate will have a valid CNA license and experience in home care or hospice settings, ensuring that clients receive essential support while maintaining their dignity. Our HHA/CNA provides paraprofessional services to the organization’s home care clients as developed by the primary nurse and as prescribed by their physician and in compliance with the state’s Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.
Position Highlights:
Follows care plan developed by RN.
Reports client changes in condition to RN, as described on care plan.
Assists clients with household tasks including, planning, and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
Measures and records client’s oral, rectal, and axillary temperatures, pulse respiration and blood pressure as ordered.
Performs range of motion and other simple procedures as an extension of therapy services as assigned.
Assists clients with use of equipment (side rails, foot stools, wheelchairs, etc.) to meet safety needs.
Performs or assists clients to perform activities in accordance with the personal care services plan, such as changing ostomy appliances or external catheter care.
Participates in multi-disciplinary case conferences as appropriate.
Accurately documents services provided in accordance with agency policy on day services are rendered.
Submits documentation in a timely manner.
Complies with agency policies and procedures.
Works with scheduling to assure accurate and up to date schedule.

Minimum Qualifications:
High School diploma or GED required.
Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
Licensed CNA within the State of Michigan
Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
Must accept responsibility for maintaining skills, learning, and adhering to agencies policies and procedures on an on-going basis.
Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics and adhere to the Compliance Program.
Must have current Driver’s license and reliable transportation to and from work site.
