As a Home Health Aide, you will implement care plans, assist with personal care and daily activities, and perform home health visits within your designated area. Accurate documentation of patient care is essential.

Responsibilities
  • Assist patients with bathing, grooming, and skin/hair care.
  • Prepare light meals and help with dressing and toileting.
  • Support with mobility, exercises, and transfers.
  • Perform household tasks and assist with medication management.
  • Document care activities accurately and timely.
  • Report changes in patient condition to the nurse or supervisor.
  • Follow the care plan and attend ongoing training.
Qualifications
  • Certified Nurse Aide (CNA) certification or active nursing student in an accredited BSN program with a GPA of 3.0 or higher.
  • Valid Driver's License and Car Insurance.
  • High School Diploma or GED.
  • Experience: 0-1 year as a Nursing Assistant or Home Health Aide, or completion of a certified training program.
Additional Information

Work hours are typically Monday to Friday, 8:00 AM to 5:00 PM, with weekend and holiday rotations. Travel using personal transportation is required. Ongoing education and training are mandatory.
