Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes according to a care plan. This role focuses on daily living and personal care tasks that support independence and comfort.

You will provide bathing, grooming, mobility support, meal assistance, and safety monitoring, communicate with the supervising nurse, document changes, and participate in Maxim's competency program. Part-time with flexible shifts and competitive salary.

Qualifications

• Meets all training requirements per state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Maintain current CPR certification as required.
• PPD or chest x-ray if applicable.
• Complete Maxim screening examinations.
• Participate in Maxim competency program.

Responsibilities

• Assist patients with plan of care including baths, grooming and daily activities.
• Take and record vitals as ordered and maintain competency.
• Support safe transfers and ambulation according to plan.
• Provide range-of-motion exercises per care plan.
• Assist with laundry and safety monitoring in the home.
• Observe and document patient condition; report changes to nurse.

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:
