Job summary

Rochester Regional Health is seeking a Home Health Aide to provide daily living assistance and personal care to patients in their homes, following the care plan and collaborating with nursing staff. The role emphasizes compassionate patient support, documentation, and adherence to HIPAA and regulatory requirements.

The position requires HHA certification or CNA eligibility with competency testing, a high school diploma, and the ability to work with minimal supervision.

Qualifications

• HHA Certification or ability to meet one of the eligibility requirements for Competency.
• Nursing student who completed Fundamentals of Nursing coursework within past 24 months and passed required tests.
• Veteran trained as medical technician or medic with required tests within 30 days of hire.
• High school diploma or equivalent.
• For ElderONE and other elderly care: 1 year experience with frail elderly population.

Responsibilities

• Patient Care & Service: Promote and restore health by completing activities in the care plan and assisting with personal care, light housekeeping, meals, and errands.
• Communication: Convey patient health status, treatment plans and progress; communicate with office and supervisory staff.
• Documentation: Accurately document client care on activity sheets.
• Compliance: Adhere to department and system protocols, HIPAA, and local, state, federal requirements.
