Job summary

Rochester Regional Health is seeking a Certified Home Health Aide (HHA) to provide care to clients in their homes, following each patient’s plan of care. You will bring compassion and attention to patients daily to promote comfort and wellbeing.

As a full-time Home Health Aide, you will work with the ElderOne Hudson PACE Center team and assist with activities of daily living, patient mobility, and essential care tasks.

Qualifications

• Certified Home Health Aide (HHA) certification preferred.
• Experience in home health care or related setting ideal.

Responsibilities

• Provide patient care and services per the care plan; assist with activities of daily living.
• Maintain effective communication with patients, families, and supervisors; document status and progress.
• Accurately document client care on activity sheets.
• Follow HIPAA, department, and system protocols and regulations.

Days: 7 am - 3:30 pm; Evenings: 3 pm - 11:30 pm; Nights: 11 pm - 7:30 am

RESPONSIBILITIES

• Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.
• Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.
• Documentation. Accurately and completely document client care on activity sheet.
• Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.

EDUCATION / LICENSES / CERTIFICATIONS

LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
