Job description

Position: Home Health Aide - Certified Nurse Aide
Hours: 40 hours per week

Schedule: 8:00 AM – 4:30 PM | Monday – Friday, with every other weekend rotation.
Location: Holland Mi | LifeCircles PACE

As a Homecare Nurse Aide, you’ll be at the heart of this mission, providing essential support to help participants maintain their health, function, and independence—all while remaining in the comfort of their homes.

What You’ll Do:

  • Deliver personal care, prepare meals, perform light housekeeping, and provide transportation as outlined in each participant's individualized life plan.
  • Support participants primarily in their homes and occasionally at the LifeCircles Day Center.
  • Assist with lifting, turning, positioning, and transporting participants using safe body mechanics and appropriate assistive devices.
  • Monitor and document participants’ emotional, behavioral, and physical changes, ensuring compliance with LifeCircles PACE policies.

What it takes:

  • Certification: Active Certified Nurse Aide (CNA) certification in the State of Michigan.
  • Experience: At least one year of experience working with frail elderly or long-term care populations.
  • Reliable transportation and a valid Michigan Driver’s License.
  • Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
