Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist clients in their homes with daily living tasks per an established care plan. You will support baths, grooming, mobility, and vital signs, and help with activities of daily living under supervision.

You will document observations, follow the care plan, and communicate clearly with clients, families, and the supervising nurse. This part-time position offers flexible hours and competitive pay, with required CPR

Qualifications

• Meets training requirements per state and federal laws when applicable.
• Ability to read, follow written instructions, and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding per plan.
• Take and record temperatures, pulse, respiration, and blood pressure when ordered.
• Safe transfer and ambulation techniques.
• Exercise according to the plan of care.
• Perform ROM and simple procedures as ordered.
• Patient laundry and care of personal items.
• Observe safety and use equipment properly (foot stools, side rails).
• Observe and teach home safety practices.
• Recognize emergencies and know emergency procedures.
• Follow plan of care and complete all tasks assigned.
• Communicate clearly with client, family/caregiver and office.
• Observe and report changes to the assigned nurse supervisor.

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
