Job Overview

Responsibilities

• Provide personal care including bathing, grooming, feeding, turning, and positioning.
• Assist with transfers, ambulation, prescribed exercises, and record vital signs as instructed by the Home Care Nurse.
• Prepare modified diets and monitor food and fluid intake/output.
• Observe and report vital signs, skin changes, and other symptoms to the RN.
• Remind patients to take medications as prescribed.
• Perform household tasks necessary for patient safety and comfort, such as cleaning and grocery shopping.
• Report any changes in the patient's physical or mental status to the nurse or coordinator.
• Complete mandatory compliance documents and in-service training annually.

Qualifications

• Completed an approved NYS Department of Health training program or enrolled as a trainee under supervision.
• Minimum age of 18, able to read/write English, and physically fit for the role.
• Have a physical exam satisfying NYS requirements for home health workers.

Compensation and Benefits

Pay: Up to $21.00/hr. Benefits include:

Work Schedule

• Various shifts including 8, 10, 12 hours, day, evening, overnight, weekends, and holidays.
