Qualifications

  • Experience in home health or long-term care environments with knowledge of medical procedures and documentation practices.
  • Strong patient observation skills to detect changes in condition promptly and accurately.
  • Experience working with individuals with disabilities or in assisted living settings is valued.
  • Knowledge of medical terminology and CPR certification to respond to emergencies.

Responsibilities

  • Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
  • Support patients with care needs like Alzheimer’s, dementia, hospice, catheter care, and disabilities.
  • Perform light housekeeping including cleaning, laundry, meal preparation, and maintaining a safe environment.
  • Document patient progress and communicate changes to healthcare teams.
  • Record observations using medical terminology to ensure accuracy and clarity.

Responsibilities
  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.

  • Support patients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.

  • Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.

  • Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.

  • Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.
