Job summary

Kanabec County is seeking a part-time Home Health Aide to assist clients with activities of daily living as designated by a registered nurse. You will be supervised by a registered nurse and may help in Public Health Nurse Clinic with foot care.

The role requires HHA certification, active MDH Nursing Assistant Registry, CPR certification, and reliable transportation to client sites. Intermittent hours and varied caseloads are expected.

Responsibilities

• Assists clients with activities of daily living as designated by the assignment sheet established by a registered nurse.
• Provides bathing, grooming, meal preparation, and light housekeeping under supervision.
• Assists with range of motion exercises, transfers, and ambulation using equipment as directed.
• Offers emotional support and encourages independence within medically determined limits.
• Reports changes to the Registered Nurse and documents services rendered in the home.
• Maintains HIPAA-compliant confidentiality and safety standards.
• Participates in required in-service programs and team case conferences.

Job Summary

The Home Health Aide is responsible for assisting clients with task of daily living as designated by the assignment sheet that is established by a registered nurse. The Home Health Aide will be supervised by a registered nurse and/or Nursing Supervisor. May also assist with Public Health Nurse Clinic providing foot care.

Some Examples of Essential Duties

95% Health Care

• Performs bathing, bedding, hair care, and other personal care as directed, under the supervision of the nurse.
• Performs range of motion exercises, assists with ambulation and transfers with use of equipment or assisting devices i.e. walkers, wheel chairs, hospital beds, etc. under the supervision of a Registered Nurse and/or physical therapist.
• Assists the patient with the activities of daily living including personal care, meal preparation, and light housekeeping activities.
• Offers emotional support and promotes the patients alertness through conversation, reading, etc.
• Performs clean dressing changes under the supervision of a Registered Nurse.
• Encourages patient and/or family to become as independent as possible within medically determined limitations.
• Provide reminders to the patient for medication, under the direction of a Registered Nurse.
• Reports any changes and/or concerns in the patients’ mental or physical condition to the Registered Nurse.
• Requests the family to call the physician in the case of an emergency and if the family is not available the Home Health Aide must call the nurse for instructions.
• Completes reports and records services rendered while in the home.
• Maintains a health, safe environment and/or as a part of an interim plan to maintain the client unit in the home.
• Maintain client confidentiality and be compliant with the Health Information Portability and Accountability Act (HIPPA).

5% Miscellaneous

• Participates in mandatory in-service programs for Home Health Aides and staff meetings.
• Attends classes, seminars, etc to maintain certification.
• Maintains CPR certification.
• Attends case conferences for team staffing regarding clients they service.
• Assist in the response to Public Health emergencies as directed by the CHS Administrator/Public Health Director
• Performs other related duties as directed.
