Position Summary:
We are seeking a compassionate and dedicated Home Health Aide (HHA) to provide essential care and support to patients in their homes. As an HHA, you will assist individuals with daily living activities, ensuring their comfort, safety, and well-being while adhering to the prescribed care plans. The ideal candidate is reliable, professional, and empathetic, with a passion for helping others maintain their independence in a home setting.

Key Responsibilities:
- Follow Care Plans: Adhere to the patient's assigned plan of care, reporting any changes or unusual conditions to the staff nurse promptly.
- Personal Care Assistance: Help patients with personal care tasks, including grooming, bathing, dressing, and maintaining hygiene.
- Household Support: Assist with light housekeeping duties such as laundry, shopping for groceries and household items, preparing and serving meals/snacks, and supporting patients with eating as needed.
- Mobility Assistance: Aid patients in transferring, moving in and out of bed or chairs, walking, and other mobility tasks to ensure their safety and comfort.
- Emergency Support: Know when and how to seek emergency assistance if required to ensure the patient's safety and well-being.
- Comfort and Safety: Provide attentive care during your shifts, ensuring patient comfort and security when caregivers are absent.
- Professional Development: Attend in-service meetings and training sessions to maintain up-to-date knowledge and skills.
- Professional Conduct: Maintain a positive, professional attitude and project a caring image, wearing the company-issued ID badge at all times.
- Technology Use: Utilize applications like Paylocity, Authenticare, MatrixCare, HHAExchange, and Carebridge as required for recordkeeping and communication.
- Other Duties: Perform additional responsibilities as assigned by supervisors or management to meet the needs of the patient and team.

Requirements
- High school diploma or equivalent (required).
- Completion of a minimum 75-hour Home Health Aide (HHA) training course (required).
- Must possess a valid driver’s license, reliable transportation and proof of state-required vehicle insurance.
- Must possess a smartphone with acceptable internet capabilities and data limits required to perform certain functions of position.
- Previous experience in home health care or a related field is preferred but not required.
- Ability to follow written and verbal instructions, and communicate effectively with patients, families, and healthcare staff.
- Compassionate, patient, and empathetic demeanor, with a commitment to providing high-quality care.
- Ability to lift, transfer, and assist with mobility tasks as needed.
- Comfortable using technology for documentation and communication.
- Must pass background checks and drug screening.
