Job summary

Ohana Pacific Health is seeking a Home Health Aide to provide personal care and assist with prescribed therapy exercises in patients' homes on the island of Kauai.

As part of an interdisciplinary team, you will help patients reach higher levels of self-care and report changes in skin condition to Case Managers. This full-time role offers competitive pay, benefits, and a $1,000 retention bonus.

Qualifications

• High School Diploma or equivalent.
• Completion of state-approved Nurse Aide Training Program.
• CNA certification preferred in Hawaii.
• Current BLS Certification required.
• 2 Step TB clearance and health review.

Responsibilities

• Provide activities of daily living (ADL) care such as bathing, hair care, nails, skin care and oral hygiene.
• Serve as an integral member of the interdisciplinary team.
• Recognize and report changes in skin condition to Case Managers.

The Position:

We're looking for passionate Home Health Aides provides personal care and assigned therapy exercises within the patient's home; assists patient in reaching the maximum level of self care.

• Provide activities of daily living (ADL) care including bathing, hair care, nail and skin care, and oral hygiene.

Requirements:

• High School Diploma or equivalent. / Minimum age of 18.
• Completion of state-approved Nurse Aide Training Program required.
• Certified Nurse Aide (CNA) certification with the State of Hawaii preferred.
• Must maintain current BLS Certification.
• Current and valid 2 Step TB clearance and health review (can be done in-house).
