Job summary

PurposeCare is seeking compassionate Home Health Aides to support clients in their homes. You will assist with daily living tasks, provide personal care, and help maintain safety and comfort for patients in their own environments. This is hands-on, patient-centered work requiring reliability and empathy.

Qualifications

• High school diploma or equivalent is required.
• Completion of a state-approved HHA training program or CNA/STNA credentials preferred.
• Relevant caregiving experience is acceptable if no formal credential.
• Basic knowledge of patient safety protocols is expected.

Responsibilities

• Provide personal care (bathing, grooming, dressing, oral hygiene).
• Assist with mobility, transfers, and range-of-motion exercises.
• Prepare meals and assist with feeding when needed.
• Support medication reminders (not administer).
• Monitor patient condition and report changes to supervising nurse.

Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

• Patients feel safe, respected, and well cared for
• On-time arrival and dependable attendance for assigned visits
• Positive feedback from patients and families
• Completion of required certifications and training on schedule
• Strong communication with supervising nurse and care team

Minimum Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Home Health Aide (HHA) training program, CNA certificate, STNA license or at least 1.5 years of recent caregiving experience in home care, assisted living, long-term care, nursing home, or hospital settings.
• HHA certification or equivalent credential as required by Ohio state regulations.
• Basic knowledge of patient care and safety protocols.
• Ability to pass required background checks and health screenings.

Preferred Qualifications

• CPR and First Aid certification.
• Familiarity with Electronic Health Record (EHR) systems.
• Strong interpersonal and communication skills.
• Experience caring for seniors or individuals with chronic conditions, disabilities, or dementia.
