Job summary

PurposeCare is a leading provider of home care and home health services in the Midwest, delivering care to more than 3,000 clients monthly. We are seeking a dependable, patient, and service-driven Home Health Aide/CNA to join our growing Cleveland-area team and support patients in their homes across our service area.

You will provide personal care, assist with daily living activities, and collaborate with a supervising nurse. Training, weekly pay, and opportunities for advancement are available.

Qualifications

• High school diploma or equivalent.
• HHA certification or equivalent credential as required by Ohio state regulations.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (no independent med administration).
• Monitor and report patient condition and changes to the supervising nurse.
• Take and record basic vital signs when trained and assigned.
• Maintain a clean, safe, and orderly client environment.
• Assist with linens and light client laundry as appropriate.
• Use safety equipment properly (rails, supports, stools, etc.).
• Document care provided according to company and regulatory standards.
• Travel reliably to patient homes and assigned visits across your service area.
• Complete required in-service training, including annual dementia education.

Minimum Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Home Health Aide (HHA) training program, CNA certificate, STNA license or at least 1.5 years of recent caregiving experience in home care, assisted living, long-term care, nursing home, or hospital settings.
• HHA certification or equivalent credential as required by Ohio state regulations.
• Basic knowledge of patient care and safety protocols.
• Ability to pass required background checks and health screenings.

Preferred Qualifications

• CPR and First Aid certification.
• Familiarity with Electronic Health Record (EHR) systems.
• Strong interpersonal and communication skills.
• Experience caring for seniors or individuals with chronic conditions, disabilities, or dementia.
