Job summary

Affirmed Home Care in Coram, NY is hiring experienced Certified Home Health Aides (HHAs) for immediate per diem assignments across Suffolk County. This role emphasizes compassionate, one-on-one in-home care tailored to each client’s needs.

You will assist with ADLs, personal care, mobility, meals, medication reminders, companionship, and maintain a safe home environment while tracking changes in condition. Weekly pay and flexible shifts offered.

Qualifications

• Current New York State Home Health Aide (HHA) certification required
• At least 1 year of professional HHA experience
• Must be authorized to work in the United States
• Valid driver’s license preferred

Responsibilities

• Assisting clients with activities of daily living (ADLs)
• Providing personal care, bathing, grooming, and hygiene assistance
• Assisting with mobility and transfers as appropriate
• Providing medication reminders as directed
• Preparing meals and assisting with light housekeeping
• Providing companionship and emotional support
• Observing and reporting changes in a client's condition
• Maintaining a safe, clean, and comfortable home environment
• Supporting clients in maintaining dignity, independence, and quality of life

HHA Job Responsibilities

As a Certified Home Health Aide, you will provide compassionate and reliable care based on each client's individual needs. Responsibilities may include:

HHA Requirements

• Dependable, compassionate, and professional approach to patient care

Required Health Documentation

Applicants must have current health documentation, including:

• Physical examination completed within the past 12 months
• PPD or QuantiFERON test completed within the past year, or a chest X-ray completed within the last 5 years
• MMR immunization documentation within the last 10 years
