Job summary

PurposeCare, a leading home health and home care provider, seeks compassionate caregivers to support clients in their homes. You will perform personal care, assist with daily living, and ensure safety and comfort during each visit.

This hands-on role offers flexible scheduling and opportunities to pursue certification if not already held. Travel across the service area is expected, with close collaboration with a supervising nurse to monitor client well-being.

Qualifications

• Active HHA or CNA certification or willingness to obtain certification.
• Ability to follow written care instructions and document services.
• Able to lift up to 25 pounds.
• Reliable transportation and ability to travel to client homes.
• TB testing, background screening and onboarding requirements.
• Compassion, patience and professionalism with vulnerable populations.

Responsibilities

• Provide personal care such as bathing, grooming, dressing, oral hygiene.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders.

• Travel to patient homes within service area.

Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

• On-time arrival and dependable attendance for assigned visits
• Positive feedback from patients and families
• Completion of required certifications and training on schedule
• Strong communication with supervising nurse and care team

Must-have requirements

• Age 18 or older
• Active HHA or CNA certification or willingness to obtain certification
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

• 6+ months of caregiving or direct patient care experience
• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
