Job summary

UNIFI Healthcare seeks a Home Health Aide (HHA) to provide essential personal care and supportive services under RN supervision. The role emphasizes safety, comfort, and independence while following the Plan of Care in clients’ homes.

Responsibilities include ADLs, meal support, documentation, transport, companionship, and maintaining infection control standards. The HHA helps promote client dignity and independence in community-based care.

Qualifications

• High school diploma or GED is required.
• Completion of an approved HHA training program.
• Current CPR/First Aid certification.
• TB screening within the past year.
• Clear state background check.
• Reliable transportation with valid auto insurance.
• Ability to lift at least 25 pounds.
• Ability to read, write, and communicate effectively in English.
• Strong communication skills and ability to follow written and verbal instructions.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, toileting, transfers, and ambulation.
• Provide meal planning, preparation, and feeding support per the Plan of Care.
• Empty catheter/colostomy bags and document output.
• Perform light housekeeping and accompany clients to appointments.
• Engage clients in recreational activities and provide emotional support.
• Document care, status, and concerns on the same day of service.
• Notify RN Case Manager of changes in client condition promptly.

Job description

The Home Health Aide (HHA) provides essential personal care and supportive services under the supervision of the RN Case Manager or designated administrator. The HHA works closely with clients and families to promote safety, comfort, and independence while strictly adhering to the established Plan of Care (POC).

Key ResponsibilitiesClient Care & Support

• Assist clients with Activities of Daily Living (ADLs), including:
• Bathing (bed bath, shower, or tub)
• Dressing and grooming
• Toileting
• Transfers and ambulation (bed, wheelchair, automobile, etc.)
• Turning and repositioning bedridden clients to prevent pressure injuries
• Provide meal planning, preparation, and feeding support according to dietary guidelines in the Plan of Care
• Empty catheter and colostomy bags and accurately document output (without changing tubing or placement)
• Change bed linens and assist with laundering soiled clothing
• Perform light housekeeping to maintain a clean, safe, and healthy environment
• Accompany clients to appointments and errands, providing transportation and companionship
• Engage clients in recreational and social activities such as walking, reading, and conversation
• Provide emotional support to clients and families

Documentation & Communication

• Accurately document client care, condition, progress, and concerns
• Complete all required documentation on the same day services are provided
• Record vital signs, meal consumption, fluid intake, and urine/bowel output as directed
• Promptly report any changes in client condition to the RN Case Manager or administrator
• Immediately report any client hospitalization (care is paused until further direction)

• Maintain infection control standards and safe home practices
• Recognize emergencies and follow agency protocols
• Protect client confidentiality and uphold ethical standards
• Practice strictly within the HHA scope of practice
• Respect clients’ Bill of Rights
• Comply with the company’s zero-tolerance drug and alcohol policy
• Participate in weekly training or continuing education sessions
