Job summary

UNIFI Healthcare seeks a dedicated Home Health Aide (HHA) to provide essential personal care and support in clients' homes under RN supervision. You will assist with ADLs, monitor condition, and promote safety and independence per the Plan of Care.

The role requires strong communication, reliability, and the ability to lift 25 pounds. You will document care, report changes promptly, and participate in ongoing training to ensure compassionate, compliant in-home care across the U.S.

Qualifications

• High school diploma or GED required.
• HHA training program completion and competency testing.
• Current CPR/First Aid certification.
• TB screening within the past year and clear background check.
• Reliable transportation with auto insurance and ability to lift 25 pounds.

Responsibilities

• Assist clients with ADLs including bathing, dressing, toileting, transfers, and mobility.
• Provide meal planning, preparation, and feeding support per care plan.
• Document care, monitor vital signs, and report changes to RN Case Manager promptly.
• Maintain infection control, safety, and ethical standards in clients' homes.
• Provide companionship, accompany to appointments, and engage in activities.

Job description

The Home Health Aide (HHA) provides essential personal care and supportive services under the supervision of the RN Case Manager or designated administrator. The HHA works closely with clients and families to promote safety, comfort, and independence while strictly adhering to the established Plan of Care (POC).

Key ResponsibilitiesClient Care & Support

• Assist clients with Activities of Daily Living (ADLs), including:
• Bathing (bed bath, shower, or tub)
• Dressing and grooming
• Toileting
• Transfers and ambulation (bed, wheelchair, automobile, etc.)
• Turning and repositioning bedridden clients to prevent pressure injuries
• Provide meal planning, preparation, and feeding support according to dietary guidelines in the Plan of Care
• Empty catheter and colostomy bags and accurately document output (without changing tubing or placement)
• Change bed linens and assist with laundering soiled clothing
• Perform light housekeeping to maintain a clean, safe, and healthy environment
• Accompany clients to appointments and errands, providing transportation and companionship
• Engage clients in recreational and social activities such as walking, reading, and conversation
• Provide emotional support to clients and families

Documentation & Communication

• Accurately document client care, condition, progress, and concerns
• Complete all required documentation on the same day services are provided
• Record vital signs, meal consumption, fluid intake, and urine/bowel output as directed
• Promptly report any changes in client condition to the RN Case Manager or administrator
• Immediately report any client hospitalization (care is paused until further direction)

• Follow the HHA Plan of Care at all times
• Maintain infection control standards and safe home practices
• Recognize emergencies and follow agency protocols
• Protect client confidentiality and uphold ethical standards
• Practice strictly within the HHA scope of practice

• Comply with the company’s zero-tolerance drug and alcohol policy
• Participate in weekly training or continuing education sessions

Qualifications

• High school diploma or GED
• Successful completion of an approved HHA training program
• Completion of required competency testing
• Current CPR/First Aid certification
• TB screening within the past year
• Clear state background check
• Reliable transportation with valid auto insurance
• Ability to lift at least 25 pounds
• Ability to read, write, and communicate effectively in English
• Strong communication skills and ability to follow written and verbal instructions
