BAYADA Home Health Care in Hershey, PA is seeking a Registered Nurse (RN) for Home Health Admissions. This role involves Start of Care Admissions for our Home Health Visits office across Dauphin and Cumberland counties, with flexible schedules and 1:1 patient care.

The ideal candidate has PA RN license, at least one year of clinical experience, and experience with EMR systems. Travel to patient homes is required; strong time management and communication skills are essential for delivering

Qualifications

• A PA RN license is required.
• Minimum one year of clinical (medical/surgical) experience.
• Graduation from an accredited nursing program.

Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform administration of medications, wound care, treatments, and procedures.
• Monitor clients' conditions; report changes to Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
