Job summary

Partners In Recovery LLC seeks an experienced RN supervisor to direct and monitor home health services, ensuring safe, compliant patient care plans and effective scheduling. The role requires leadership, strong communication and adherence to HIPAA regulations in a busy home health setting.

The ideal candidate holds an active Virginia RN license, can supervise clinical staff, and is capable of coordinating care across offices and client homes. On-site presence in Reston is required.

Duties and Responsibilities:
  • Directs the development, implementation and maintenance of patient treatment plans.
  • Oversees the patient care planning process and ensures that individual treatment plans reflect quality and safe delivery of home health care services.
  • Directs and monitors the scheduling process and ensures that visit frequency is altered to meet changing patient needs.
  • Ensures the accuracy and completeness of all clinical aspects of patient case records.
  • Maintains confidentiality of patient and personnel information within HIPAA guidelines.
  • Ensures that appropriate infection control and safety measures are implemented, and all hazards and accidents are reported.
  • Receives, reviews and responds to all incident reports and client complaints including written reports and follow-up.
  • Ensures that use of equipment and supplies by Agency staff members is safe and efficient.
  • Performs nursing functions in emergency staffing situations and as needed and participates in the on-call rotation when necessary.
  • Monitors the referral intake process and acts as a liaison between referral source, physical, Agency staff and patient.
  • Ensures physical availability of licensed nurse onsite at all times during operating hours according to federal and state standards.
  • Maintains own (or alternate) availability on site or immediate availability to be on site during operational hours and additionally as needed.
  • Monitors Agency staff member's performance for compliance with appropriate policies and procedures regarding patient rights.
  • According to Agency policy, supervises the TB screening and testing for all at risk patients care personnel prior to employment and annually thereafter.
  • Makes schedules for on-call rotation for nurses and oversees nurse's productivity reports.
  • Maintains appropriate staffing levels of qualified personnel, oversees the clinical orientation and in-service processes and conducts staff evaluations and other personnel functions to ensure the staff meets the highest standards of quality.
  • Interviews applicants and makes recommendations for hiring of personnel.
  • In conjunction with HR ensures qualifications/certifications of all clinical personnel is performed prior to employment and annually thereafter.
  • Conducts and document disciplinary action, performance reviews, performance improvement plans of clinical staff members per Agency policy annually.
  • Coordinates, schedule and conduct clinical orientation and in services training of clinical staff to promote health and safety of patient, familiarize staff with regulatory issues and Agency policies and procedures.
  • Assures accurate, complete and current personnel files on clinical staff are maintained.
  • Assures that clinical competency and records to support competency of all allied health personnel is performed according to Agency policy.
  • Refers impaired personnel to appropriate authorities including licensing bodies.
  • In conjunction with Administrator, develops, implements, communicates and enforces written policies and procedures that reflect Agency philosophy and objectives and support higest qualigy standards of patient care, cost controls, qualigy improvement and mechanisms for disciplinary acion; and promotes effective communication.
  • Submits written reports and provide assistance to the Administrator as required.
  • Fulfills additional responsibilities as assigned and as necessary.
Qualifications:
  • Excellent Oral and written communication skills.
  • Excellent administrative, organizational, and leadership skills.
  • Must be dependable and have a professional work ethic.
  • Ability to work independently and as a contributing team member.
Experience:
  • Minimum one year's experience in Home Health with three to five (3-5) years supervisory experience in a hospital, nursing home, public health or home health.
  • Home Health experience is preferred.
Education:
  • Graduate of an accredited school of nursing. Currently licensed as an RN with no restrictions in the state of practice.
  • Ability to read and write English. Basic computer skills preferred.
Licenses:
  • Valid State of Virginia Driver's License with good driving record.
  • Current Registered Nurse (RN) license.
  • Maintains professional licensure.
Physical Requirements:
  • Ability to lift 35 - 45 pounds.
  • Use hands to grip or feel.
  • Reach with hands and arms.
  • Occasionally required to bend, stoop and kneel.
  • Frequently required to stand on one's feet for a long period of time.
  • Ability to deal effectively with stress.
Working Conditions:
  • Works in office areas as well as in client homes and assisted and independent living facilities. Interacts with clients, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances.
  • Is subject to hostile and emotionally upset clients, family members, staff, visitors, etc. under all conditions/circumstances.
  • May be exposed to infectious waste, diseases, conditions, etc.

Summary

To ensure quality and safe delivery of all home care services, to direct and monitor the provision of all home care services in accordance with the Agency philosophy and established standards of home and nursing practice and in accordance with Federal, State, and accrediting regulations.
