Qualifications

• One year of experience in home health care, hospital, clinic, or nursing home preferred.
• Experience/Education in Dietetics, Nutrition, Home Health, or Nursing preferred.
• One year of experience as an LPN, DTR, or CNA preferred.
• Excellent human relations skills to interface with doctors, other referral sources, and patients.

Responsibilities

• Provide patient education as part of patient care according to patient need and policy.
• Perform complete and professional patient assessments according to policies and procedures.
• Maintain current knowledge of enteral home care techniques and enteral therapy concepts.
• Represent the company through professional appearance, patient care activities, and written reports.
• Gathers information from referral sources to ensure insurance coverage for each enteral patient; follow up on necessary paperwork for payment.
• Maintain patient records in complete and organized manner in compliance with accreditation standards.
• Drive to admit, deliver, and pick up enteral supplies to patients' home or place of business.
• Provide patient care, treatment, and service in a continuous and coordinated manner.
• Promote safe, effective patient and organizational environments and feeding equipment use.
• Participate in ongoing education including in-services and training to maintain competence.
• Offering $16.00 - $23.00 per hour
• Read and interpret safety rules and instructions.
• Calculate figures and amounts related to commissions and measurements.
• Solve practical problems with limited standardization.
• Interpret various written, oral, or diagrammatic instructions.
• Knowledge of word processing, spreadsheets, and email.
