Rendevor Dialysis is seeking a Dialysis Home Therapies Registered Nurse (RN) to support our growing home dialysis program in the Chicago metropolitan area. You will educate, train, and support patients performing dialysis at home, delivering high-quality patient-centered care with strong clinical and communication skills.

The role requires clinical expertise in hemodialysis, home therapies, and the ability to travel locally to patient homes, with adherence to CMS and regulatory standards.

Qualifications

• Graduate of an accredited School of Nursing.
• Minimum 12 months experience in hemodialysis, including home hemodialysis.
• Current CPR/BLS certification.
• Must meet state licensure requirements.
• Willingness to travel within ~30-mile radius of Chicago.

Responsibilities

• Train patients and caregivers on home dialysis procedures.
• Educate on infection control and emergency protocols.
• Assess patient suitability for home dialysis.
• Develop individualized care plans with the interdisciplinary team.
• Document accurately per CMS guidelines.
• Coordinate equipment maintenance and supply management.
• Monitor patient progress and adjust plans accordingly.
• Ensure compliance with policies and quality standards.
