Job summary
Rendevor Dialysis in Chicago is seeking a Full-Time Dialysis Home Therapies RN to support a growing home dialysis program across the Chicago metropolitan area. You will deliver patient-centered care, educate and train patients who perform dialysis at home, and work closely with a multidisciplinary team.
The role requires a nursing degree, at least 12 months dialysis experience (including home hemodialysis), CPR/BLS, and the ability to travel.
Qualifications
- Graduate of an accredited School of Nursing.
- Minimum 12 months current experience in hemodialysis, including home hemodialysis.
- CPR BLS Certification.
- Must meet state/facility requirements.
Responsibilities
- Train patients and caregivers on home hemodialysis procedures, including equipment setup and troubleshooting.
- Provide education on infection control and emergency protocols.
- Develop individualized care plans with the interdisciplinary team.
- Maintain accurate documentation compliant with CMS guidelines.
- Travel to patients' homes within the Chicago area for training and assessments.
- Monitor patient progress and adjust care plans as needed.
- Ensure patient safety and adherence to clinical standards.
