Dialysis Clinic, Inc. in New Mexico is seeking a Peritoneal Dialysis Home Therapies Nurse to coordinate the administrative and clinical aspects of the home dialysis program, including treatment options and communication with patients and staff to ensure high standards of care.

This full-time role features three 12-hour shifts starting at 4:30am with on-call duties, opportunities for growth, comprehensive benefits, and a mission-driven culture dedicated to patient-centered care.

Qualifications

• Excellent communication skills and a desire to teach (must communicate clearly with patients and staff).
• Demonstrated clinical excellence in dialysis or acute care settings.
• Strong leadership capabilities to guide care teams and coordinate home therapies.
• Desire to collaborate with care teams across nursing, physicians and educators.
• Ability to problem solve and adapt in a dynamic home dialysis program.

Responsibilities

• Collaborate with the medical team on patient care concerns, hospitalizations, discharges and modality changes.
• Coordinate home training sessions, clinic visits, patient care conferences, home visits and educational activities to maximize nursing hours and program efficiency.
• Monitor patient supply ordering and usage while maintaining quality care.
• Communicate regularly regarding home program activities with interdisciplinary care team.
• Monitor ongoing patient care parameters and intervene as needed.
• Perform all registered nurse functions and duties.
• Create and maintain documentation in accordance with ESRD Network, regulatory requirements and facility policies.
