Job summary

Herewith in Downers Grove, IL is seeking a compassionate Care Partner to support an in‑home dialysis patient during a major care transition. This role provides one‑on‑one assistance, builds rapport, and helps ensure treatment is performed safely and on schedule.

Training will be provided by our clinical team, and you will work with a dedicated care network while maintaining independence with a single client each week.

Qualifications

• 1+ year dialysis experience in home or clinic.
• Familiarity with home dialysis systems.
• Willing to complete training.
• Patient, steady, and supportive.
• Strong communication with clinical team.

Responsibilities

• Assist with dialysis treatment setup (Tableu or NxStage dialysis machine).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
