Job summary

Herewith is seeking a compassionate Care Partner to support a patient transitioning from clinic to home dialysis in Downers Grove, IL. You will work one-on-one with the patient, receiving training and ongoing clinical team support to ensure confidence in home-based treatments.

The role offers in-home care with scheduled hours, 5–6 days per week, and a starting ASAP. Compensation is $25 per hour, with training provided to ensure smooth patient transitions.

Qualifications

• 1+ year of dialysis experience
• Strong communication and teamwork with clinical staff
• Willing to complete provided training
• Open to in-home dialysis support
• Steady and patient during transitions

Responsibilities

• Assist with dialysis treatment setup and environment
• Keep space clean, organized, and safe
• Provide companionship and emotional support
• Help keep patient on treatment schedule
• Provide light ADL support
• Monitor safety and report concerns
• Maintain simple documentation and notes
