Job summary

Herewith is seeking a compassionate Care Partner in Oak Brook, IL to support a patient transitioning to home dialysis. You’ll provide one-on-one care, assist with treatment setup, and offer emotional reassurance within a stable, client-focused schedule.

Training will be provided by our clinical team to ensure confidence with home dialysis processes, equipment setup, and safety protocols. This role emphasizes patient dignity, independence, and reliable weekly hours with a single client.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Willing to complete provided training on home dialysis.
• Familiarity with home dialysis systems preferred.

Responsibilities

• Assist with dialysis treatment setup for home dialysis.
• Keep the treatment environment clean, organized and safe.
• Offer companionship and emotional reassurance to patient.
• Help the patient stay on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
