Herewith is seeking a caring Care Partner in Norcross, GA to support a patient transitioning to home dialysis. The role involves setup, safety, companionship, and documentation for a single client in the home setting.

Training will be provided by our clinical team to ensure confidence in patient-specific dialysis routines and home setup. This is a stable, consistent schedule with a supportive care network to back you up.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus).
• Open to in-home dialysis support and willing to complete the provided training.

Responsibilities

• Assist with dialysis treatment setup (NxStage or Tablo dialysis machine).
• Keep the treatment environment clean, organized, and safe.
