Job summary

SYNERGY HomeCare of Palm Bay is seeking a dedicated Home Caregiver (HHA/CNA) to provide patient-centered care in clients' homes across Palm Bay and surrounding areas.

You will assist with daily living activities, monitor medical conditions, and offer emotional support while maintaining accurate records and communicating with families and healthcare teams.

Qualifications

• Current certification as a Home Health Aide (HHA) or CNA.
• High school diploma or equivalent; college or training a plus.
• Ability to lift and transfer clients safely and assist with ambulation.
• Excellent communication and interpersonal skills.
• Reliable transportation and willingness to travel to client locations.

Responsibilities

• Assist with daily living activities (bathing, dressing, grooming).
• Monitor and manage medical conditions (wound care, meds).
• Provide emotional support and companionship to clients and families.
• Maintain accurate records and communicate with clients, families, and healthcare teams.
