Job summary

SYNERGY HomeCare is seeking a compassionate Caregiver to support clients in their homes and help them maintain independence and dignity. This role emphasizes one-on-one care and personalized support to integrate clients into their communities.

You will follow individualized care plans, provide companionship, and assist with daily living activities, transportation, and well-being monitoring. Training is provided, with options for full-time, part-time, or per diem schedules.

Qualifications

• High School Diploma or GED required.
• At least 18 years of age or older.
• Prior home health aid experience preferred.
• Valid Driver’s License and reliable transportation.
• CPR and First Aid certifications required.
• HHA license preferred.

Responsibilities

• Assist with daily living activities such as personal hygiene, meal preparation, and mobility.
• Follow ICPs tailored to each client's needs.
• Provide companionship, emotional support, and social engagement.
• Assist with medication reminders and monitor well-being.
• Support clients with transportation to appointments and community outings.
• Maintain accurate care documentation and follow care guidelines.
