Job summary

Apex Rehab Group is seeking a licensed Speech Language Pathologist for home care in Westchester County, NY. The role involves meeting patients at home, assessing speech needs, and creating personalized treatment plans to help patients reach goals.

Responsibilities include evaluating patients, implementing treatment plans, documenting care, and collaborating with families. Flexible schedule options (part-time, full-time, or per diem) are available.

Qualifications

• Active NY state license required for practicing speech therapy.
• Strong communication and interpersonal skills.
• Excellent organizational skills.
• Strong charting and documentation skills.
• Prior home care experience preferred but not required.

Responsibilities

• Provide therapy services to patients in their homes.
• Make evaluations and create treatment plans for patients’ speech and wellness goals.
• Assist in implementing treatment plans and adjust as needed.
• Maintain thorough records of patient needs, care and outcomes.
• Collaborate with patients and families to ensure understanding and success.
