New York Community Home Care is seeking a dynamic SLP Clinical Fellow for home-based pediatric cases in Brooklyn or the Bronx. The role offers a base salary of $80,000 per year plus generous benefits, including medical/dental insurance and a 401(k) after 3 months, plus 27 PTO days and 10 holidays.

Weekend caseload options and door-to-door car service are available. Positions can be part-time or full-time, with a personal driver option for commuting to cases.

Responsabilités

• Evaluates under the Speech Supervisor the Speech and Language Rehabilitation needs of the patients and provides services as ordered.
• Performs clinical tests under the Speech Supervisor to assess disorders and to develop a plan of care.
• Performs Speech Language Pathology procedures under the Speech Supervisor to meet goals.
• Instructs patients and families in the use of speech modalities and procedures.
• Maintains records for each patient treated and documents progress and discharge summaries.
• Charts visit contacts and findings within required timeframes and per agency policy.
• Communicates with the nurse regularly to discuss patient progress and re-evaluates as needed.

New York Community Home Care is looking for a part-time and full-time dynamic SLP Clinical Fellow for our Home based pediatric cases in Brooklyn or the Bronx. This position offers a base salary of $80,000.00 plus an excellent benefits package which includes: Medical and Dental insurance, 401k Plan after only 3 Months of Employment, 27 Paid Time Off Days, 10 Holidays. The Option to cover Caseload on Weekends is also available.

Only 25 Visits Weekly!

Our part-time and full-time positions offer therapists the option to have a Personal Driver pick you up from your home ( Brooklyn, Queens, Manhattan and the Bronx ) and drive you to your cases and then back home. Currently, we are the only agency in New York that offers this unique door-to-door car service!

SLP Clinical Fellow Job Description

1. Evaluates under the Speech Supervisor the Speech and Language Rehabilitation needs of the Patients and provides Speech and Language Rehabilitative Services for Patients as ordered by the Patient’s Authorized Medical Practitioner.

2. Performs clinical tests under the Speech Supervisor to assess Speech and Language disorders and potential for restoration as well as developing a Plan of Care with restorative and/or maintenance goals to meet identified needs.

3. Performs Speech Language Pathology procedures under the Speech Supervisor to achieve identified goals.

4. Instructs Patients/Family and other appropriate persons/health team personnel in the use of assistive modalities and appropriate Speech and Language Procedures.

5. Maintains records for each Patient treated by documenting general information, special precautions, description of and number of treatments rendered, reaction to treatment, progress or lack of progress made and Discharge Summary.

6. Charts all visit contacts and findings on designated forms, within twenty-four (24) hours, and as required by agency policy.

8. Communicates with the Nurse at least every two (2) weeks to discuss the Patients progress.

9. Re-evaluates Patients as needed to detect progress attained and completes a written progress report for the Patient’s record containing Patient’s progress, continued treatment plan, goals, frequency of visits and outcomes of care provided.

Performs other related duties assigned by Supervisor. In an emergency, performs other duties necessary for the welfare of Patients or the efficient operations of the Agency.
