Job summary

New York Community Home Care is seeking an SLP Clinical Fellow for home-based pediatric cases in Queens. The role offers a base salary of $80,000.00 with medical/dental, 401k after 3 months, and generous PTO.

A weekend caseload option and a personal driver service are available to commute from Brooklyn/Queens/Manhattan/Bronx. Positions are offered as part-time or full-time with tasks including evaluation, planning care, and patient/family instruction under supervision.

Qualifications

• Evaluates under the Speech Supervisor the Speech and Language Rehabilitation needs of patients.
• Provides Speech and Language Rehabilitative Services as ordered by the Medical Practitioner.
• Performs clinical tests under the Speech Supervisor to assess disorders and restoration potential.
• Develops a Plan of Care with restorative and/or maintenance goals.
• Instructs patients/family in use of speech procedures and assistive modalities.
• Maintains patient treatment records and discharge summaries.

Responsibilities

• Communicates with the nurse every two weeks to discuss patient progress.
• Re-evaluates patients as needed and writes progress reports with goals and frequency.
• Performs other duties as assigned by the Supervisor and, in emergencies, supports patient welfare.
• Charts visits and findings within 24 hours as required by agency policy.

New York Community Home Care is looking for a part-time and full-time dynamic SLP Clinical Fellow for our Home based pediatric cases in Queens. This position offers a base salary of $80,000.00 plus an excellent benefits package which includes: Medical and Dental insurance, 401k Plan after only 3 Months of Employment, 27 Paid Time Off Days, 10 Holidays. The Option to cover Caseload on Weekends is also available.

Only 25 Visits Weekly!

Our part-time and full-time positions offer therapists the option to have a Personal Driver pick you up from your home ( Brooklyn, Queens, Manhattan and the Bronx ) and drive you to your cases and then back home. Currently, we are the only agency in New York that offers this unique door-to-door car service!

SLP Clinical Fellow Job Description

1. Evaluates under the Speech Supervisor the Speech and Language Rehabilitation needs of the Patients and provides Speech and Language Rehabilitative Services for Patients as ordered by the Patient’s Authorized Medical Practitioner.

2. Performs clinical tests under the Speech Supervisor to assess Speech and Language disorders and potential for restoration as well as developing a Plan of Care with restorative and/or maintenance goals to meet identified needs.

3. Performs Speech Language Pathology procedures under the Speech Supervisor to achieve identified goals.

4. Instructs Patients/Family and other appropriate persons/health team personnel in the use of assistive modalities and appropriate Speech and Language Procedures.

5. Maintains records for each Patient treated by documenting general information, special precautions, description of and number of treatments rendered, reaction to treatment, progress or lack of progress made and Discharge Summary.

6. Charts all visit contacts and findings on designated forms, within twenty-four (24) hours, and as required by agency policy.

8. Communicates with the Nurse at least every two (2) weeks to discuss the Patients progress.

9. Re-evaluates Patients as needed to detect progress attained and completes a written progress report for the Patient’s record containing Patient’s progress, continued treatment plan, goals, frequency of visits and outcomes of care provided.

Performs other related duties assigned by Supervisor. In an emergency, performs other duties necessary for the welfare of Patients or the efficient operations of the Agency.
