Job summary

BayCare Home Care in Largo, FL is seeking a Specialty Pharmacist to provide comprehensive pharmaceutical care to patients with complex or chronic conditions requiring specialty medications. You will perform clinical reviews, patient counseling, adherence monitoring, and coordinate with prescribers to ensure safe and effective medication use.

The role requires FL licensure, a PharmD preferred, and prior specialty or clinical pharmacy experience.

Qualifications

• Pharmacist licensure in Florida (State of FL) is required.
• Pharmacist degree required with PharmD preferred.
• Experience with specialty or clinical pharmacy is preferred.

Responsibilities

• Provide comprehensive pharmaceutical care for patients on specialty meds.
• Conduct clinical reviews and patient counseling.
• Monitor adherence and coordinate with prescribers.
• Ensure safe and effective medication use and regulatory compliance.
• Support URAC and ACHC accreditation efforts.
• Collaborate with pharmacists and technicians to dispense medications.
