Personal Touch Home Care is seeking a compassionate Speech Language Pathologist for Jamaica, Queens to join our home-based, fee-for-service program. You will evaluate and treat speech, language, and communication disorders in patients at home.

You will conduct assessments, develop individualized treatment plans, and provide therapy across pediatric, adult, and geriatric populations, collaborating with caregivers and other clinicians to ensure holistic care.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Demonstrates good verbal and written communication and organization skills.
• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.

Responsabilités

• Conduct comprehensive assessments of speech and language abilities.
• Develop and implement individualized treatment plans tailored to each patient's needs.
• Provide therapy to individuals of all ages, addressing issues such as articulation, fluency, voice, and language.
• Collaborate with other healthcare professionals to provide holistic care.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate patients and their families about communication strategies and home exercises.
• Stay current with best practices and advancements in speech therapy.
